Psychiatrist
Born and raised in Puerto Rico, Dr. Xiomara Nieves-Alvarado discovered her passion for mental health at an early age. Fascinated by the connection between thoughts, behaviors, and emotional well-being, she found herself naturally drawn to listening to others and helping them navigate life’s challenges. That curiosity eventually led her to pursue a career in medicine and specialize in psychiatry, where she could combine science, empathy, and advocacy to help people achieve healthier, more fulfilling lives.
Dr. Nieves-Alvarado earned her medical degree from Universidad Central del Caribe School of Medicine in Bayamón, Puerto Rico, before completing her psychiatry residency at Duke University Hospital. As a board-certified psychiatrist fluent in both English and Spanish, she has extensive experience treating anxiety, depression, PTSD, OCD, substance use disorders, and other complex mental health conditions. Her diverse training includes forensic psychiatry, veteran care, family therapy, medically assisted treatment, and electroconvulsive therapy. She was also nominated for a psychodynamic fellowship through the Psychoanalytic Center of the Carolinas and remains actively involved in academic education and professional organizations.
Today, Dr. Nieves-Alvarado provides telehealth psychiatric care at Talkiatry to patients across Florida, North Carolina, New York, and Rhode Island. Her philosophy is rooted in collaboration, evidence-based treatment, and one simple but powerful principle: listening. Patients consistently describe feeling heard, validated, and respected. She combines medication management with insight-oriented therapy, believing that personalized, comprehensive care creates the strongest foundation for long-term mental wellness.
A decade after college, Dr. Nieves-Alvarado reunited with an old friend, found love, and made Orlando her home- quickly embracing its vibrant community, cultural diversity, and welcoming spirit. When she is not caring for patients, she enjoys traveling, supporting local artists and vendors, exploring Orlando’s culinary scene, and spending time with her family and beloved dog. For Dr. Nieves-Alvarado, success is measured by every patient who can once again sleep peacefully, find joy, and fully engage in life.
